As a Foster/Proctor Care applicant, you are required to:
- Be at least 21 years of age
- Be in physical and mental health sufficient to comply with agency health requirements.
- Significant personal relationship must be of one year’s duration.
- Complete financial statement satisfactory to the Agency that the applicant’s income is sufficient enough to meet the applicant’s family needs.
- Provide the names and addresses of at least four character references.
- Have no serious criminal offense history.
- Attest whether or not minor children or house mates in the home have a record of serious law violations.
- Be able and willing to cooperate with agency requirements regarding administrative management of the program
- Agree to participate in yearly training requirements.
- Be able and willing to cooperate with Agency representatives in managing clients case and care.
